  • Where you saw the physician the last time, and where your medical file is located,
    
    
  • Let us know if your last visit to this physician was more than 5 years ago,
    
    
  • Let us know if you don't have a family doctor or were treated at a walk-in clinic.
    
    This makes it easier for us to locate the attending physician and retrieve the required medical information.
