Tips
on how we can work together to further speed up the APS process:
- At
the time of application please give us as much information as you can:
- Accurate physician's name,
- Full name of the applicant
(including previous names, maiden names or aliases),
- Where the applicant
was seen by the physician/location of the medical file,
- Keep in mind
that some doctors work out of more than one location, e.g. hospital,
medical clinic and private practice. Give us the
location where the applicant has seen the doctor.
- Let us know if the applicant doesn't
have a family doctor or was treated at a walk-in clinic.
This makes it easier for us
to locate the attending physician who will complete the report.
- When
